The full rate-limit decision for one request (DW-041): the LIVE enforcement outcome (exactly the pre-dry-run check semantics over live bundles) plus the first dry bundle’s would-be denial, if any evaluated to one.

§outcome: RateLimitOutcome§dry_denied: Option<RateLimitOutcome>

The first (most specific) dry_run bundle that would have denied this request: a RateLimitOutcome::Denied payload for the log line and metric — never enforced, never on the response.
