Emit one access-log event (one JSON line under the binary’s subscriber). The field list is exhaustive and redacted by construction: no headers, no query string, no credentials. bytes_in/bytes_out (PERF-12, #209): bytes_in is the declared Content-Length (0 for chunked); bytes_out is the frame-counted response body size (read from the atomic the CountingBody wrapper updated as frames streamed to the client).
