Default pending-request cap when max_pending is absent (#270).

Previously absent = unbounded queueing, which allowed memory to grow without limit when the connection cap was saturated. A default of 256 bounds the queue so excess requests get 503 “upstream saturated” (backpressure) instead of queueing indefinitely. This aligns with the AGENTS.md rule that buffering must be size-capped. Operators who need deeper queueing can set max_pending explicitly.
