Default connection cap when connection_cap is absent.
Raised from 64 to 256 (#270): the previous default of 64 caused
unbounded memory growth under high concurrency — excess requests
queued on the semaphore with their full request context held in
memory, and the allocator retained those pages. 256 is a reasonable
middle ground for modern workloads; operators with higher
concurrency needs should set connection_cap explicitly.
